NEW SKYWARD FORM – PERMISSION FOR NON-EMERGENT CARE!
Parents, if you have already filled out all the Skyward forms, be aware that there is a new one that was just added that you need to fill out. This relates to a new state law that just went into effect on July 1, 2024. The new law requires parents or guardians to give active consent for ALL non-emergency medical care given at school. So, if your child falls and skins their knee on the playground, the nurse can’t put a band-aid on without filling out this form ahead of time.
The form is now available on Skyward. Log back in and fill it out for each of your children. It is straightforward and only 4 steps.
1.) Log in Skyward from parent portal. From the main screen, you should see “An Online Form is now available to fill out.”
2.) Click “Fill out Online Form for …”
3.)Select Yes in the Permission for non-emergent care if you wish for your child to receive care at school. Sign the form and click “Complete Step 1 and move to Step 2”
4.)Click “Submit Non-Emergent Care Form”
