Conditional Logic: New questions or instructions “pop up” basic on certain answers. For example if you select “Elementary School”, Grade Level Question pop up.
Email Notification: Email notifications can be sent to one or more addresses based on answers to the questions. For example, if you answer “Yes” to any of the symptom questions, an email can be sent to the school nurse.
Customized Confirmaions: Different pop ups, links and landing pages can display based on given answers. Parent instructions, links to health organizations.
Reports: Summary reports are created as CSV and emailed daily to teachers at a specific time. Report includes those who have completed the report only. Teachers cross reference report with classroom list to determine who needs a health screening test.
Archived Entries: Entries can be stored and retrieved in report format as long as needed.
Mobile and Webpage Views: Survey is available on Mobile App and via Website link we will provide.
